The Pee Dee LWIA offers the following answer to a question received December 7, 2009.

 

Question: "Under Section O of the grant request, we are asked to provide an invoice for Fidelity Bonding showing coverage for the current year. Since my organization does not currently have a Fidelity Bond, I will need to obtain one.  But, upon speaking with an agent, he said that he would need a Bond form in order to know what type of coverage I would need.
 
Can you provide me with a Bond form?"

Answer:  Pee Dee LWIA staff is not familiar with the Bond form that the insurance agent is requesting.  However, a complete description of the bonding requirements is included in Section 3.5.7 of the GAR terms and conditions (Attachment E).
